Holly Prokopy 47, of Elm St Woonsocket passed away at Landmark Medical Center on November 3rd, 2015. Holly was the daughter of Warren and Eleanor Weaver. She is survived by her loving husband Kent Prokopy and their two adoring children Christopher 28 and Megan 18 all of Woonsocket.
The family will celebrate her life at a private gathering.
